What To Expect During a Typical First Visit

Depending on your reason for a visit — whether it has dental or esthetic focus— we will first invite you to complete the necessary new-patient forms on a dedicated tablet: The first is a questionnaire about your oral and relevant medical history. The second — known as HIPAA dental form—outlines your rights on the use and disclosure of your protected health information, as well our responsibilities in safeguarding it.  

If you wish, you may complete these forms in your appointment confirmation email before you arrive.
